Medical Form Completion
Gap Year Forms
Students taking a gap year before University are often required to have a medical fitness form completed. We can usually help out but pricing is bespoke because the form requirements can vary widely. Prices start from £90 and will always involve a face to face medial assessment.
In the first instance, please email us a copy of the form to ensure it is appropriate for a private GP service to complete (some forms require a GP who access to the full NHS notes). We can then give you a decision and quote a price.
Prior to attending your appointment please obtain an up to date printed NHS Medical Summary from your GP surgery which the NHS receptionist should be able to print off fairly easily. This should include the following information:
- Past medical history
- Medication history
- Allergies
- Immunisation history
- Last three NHS GP consultations
This will allow our private GPs to ensure that we are recording accurate and up to date information on your medical form.

Please email a copy of any other form requirements to our reception team and they will be happy to let you know whether we can help. Please note that since DVLA forms and Taxi driver forms usually require access to full NHS GP records we are unable to complete these.
